Managing transplant recovery through virtual, postoperative care

Care Innovations Guide

Meet Clarence

Clarence is a stay-at-home kind of person. He enjoys walking his dog, playing cards, and simply sitting with his family at his lakeside house. Just two years ago, Clarence began showing signs and symptoms of liver dysfunction. As the symptoms progressed, his doctors advised Clarence to have a liver transplant.

After undergoing transplant surgery, he began his postoperative recovery in the hospital. Liver transplant patients spend an average of 28 days in the hospital. Clarence’s stay was shorter. That’s because his physician enrolled him in a study using the Care Innovations™ Guide a remote care management solution—as a new model of care for transplant patients. The Guide was part of his care in the immediate postoperative period at the hospital. Once at home, he continued using the Guide so he could be constantly connected to his healthcare team.

Clarence’s nurses and physicians were able to regularly check his general condition, collect biometric data, manage his treatment and offer face-to-face conversations via video conferencing. The healthcare team was also able to track dosages of his immunosuppressive medications. With this regular, daily monitoring, his care team always had the information they needed to act sooner, where necessary, and Clarence avoided a hospital readmission.

While patients recuperate from liver transplantation differently, Clarence was able to do all of his follow-up from home, instead of having to travel back to the hospital. Being part of this model of care study improved his quality of life tremendously. He benefitted from having the comforts of his own home, family members around him, and minimal trips out of the house that could subject him to infection.

Clarence is healthy, and back to his hobbies. Learn more about the Care Innovations Guide and its role in virtual, postoperative care.
