The Virtual Care Suite uses a “management by exception” model so that healthcare professionals can more accurately prioritize clinical needs and provide quality healthcare remotely. This design allows clinicians to set parameters to automatically identify outliers and flag people that should receive proactive intervention by a healthcare professional. As part of the Care Innovations Guide platform, the Care Innovations™ Guide in-home care management client application facilitates daily biometric data collection and transmission, interactive education delivery, and regular contact with healthcare professionals from a patient’s home.

The in-home solution features videoconferencing, an intuitive touchscreen interface, and support for capturing biometric data through peak flow meters, weight scales, pulse oximeters, glucose meters, blood pressure monitors, and other input devices. By capturing biometric data on a daily basis, a patient can provide the necessary information to help their providers and healthcare professionals understand daily health status and behavior in order to identify and overcome individual challenges and provide solutions for care.
