Care Innovations™ Guide

The Care Innovations™ Guide platform enables healthcare providers to offer a clinically driven, fully integrated remote care management solution for populations with chronic conditions. The Guide platform serves as a foundation to provide unique, differentiated, and branded in-home solutions that deliver more frequent touch points between clinicians and patients. These touch points help engage patients in self-directed, active management of their disease and keeps all members of the care team apprised of the patient’s condition.

Enterprise-Class Clinical Care Management Application

The Care Innovations™ Guide–Virtual Care Suite, an enterprise-class clinical care management application, lets clinicians connect with patients in their homes, provides information that may bring attention to potential health issues before they grow more serious, and helps clinicians prioritize care protocols based on each person’s individual needs. The Guide–Virtual Care Suite (VCS) uses a management-by-exception model so that healthcare professionals can more accurately prioritize a patient’s clinical needs and remotely provide quality healthcare. Comprehensive In-Home Patient Application

The Guide platform’s in-home patient application can be deployed in two ways. The new cloud-based patient application, Care Innovations™ Connect RCM, provides care recipients with easy access to the Guide platform from anywhere using their own web-connected device.1 The in-home application helps healthcare providers facilitate daily biometric data collection and transmission, interactive education delivery, and regular, remote contact with care plan members. As an alternative to the Connect RCM application, the Guide platform can be accessed by patients using a single-purpose tablet installed in the home and provided by a healthcare organization. Once activated and enrolled as a member of a remote care management program using Connect RCM, the care recipient also has access to the Care Innovations™ Connect Caregiver application. Connect Caregiver is an easy-to-use suite of tools that enables care recipients to invite informal caregivers to a care circle.

Powerful Data Integration Engine

The Guide platform includes the Care Innovations™ Guide–Data Bridge, a powerful data integration engine that enables the data collected remotely from the patient to be integrated into existing electronic medical record (EMR) systems and other clinical information systems (CIS). The Guide-Date Bridge enables biometric and behavior data collected by the Guide platform to be combined with data from medical claims, pharmacy claims, labs, and physician reports for record-keeping and analysis.

The Virtual Care Suite (VCS) uses a management-by-exception model so that healthcare professionals can more accurately prioritize a patient’s clinical needs and remotely provide quality healthcare. This design allows clinicians to set parameters to automatically identify outliers and flag patients that should receive proactive intervention by a healthcare professional. From the VCS dashboard, clinicians can quickly view reports on participants’ progress, examine full histories of biometric data measurements captured from home, set reminders to direct care activities, and schedule regular health assessments.

The VCS is a secure, cloud-based, hosted application that can be easily scaled from initial deployment through enterprise-wide deployment to meet the needs of your growing populations. The use of the Guide–Virtual Care Suite improves clinician efficiency, and a model of care led by a clinician using the Guide platform has been shown to:

For healthcare providers, the in-home solution helps facilitate daily biometric data collection and transmission, deliver interactive education delivery, and initiate regular, remote contact with care plan members. For patients, the application features an intuitive user interface, support for capturing biometric data through a variety of input devices, videoconferencing, and helpful self-management care tools. By capturing biometric data on a daily basis, a patient can provide the necessary information to help their providers and healthcare professionals understand daily health status and behavior in order to identify and overcome individual challenges and provide solutions for care.

The Guide platform’s new cloud-based patient application, Care Innovations™ Connect RCM, makes clinically driven remote care easier and more accessible than ever before. With Connect RCM, the process of getting care recipients up-and-running on their remote care plan is quick and easy.

As a cloud-based client application, Connect RCM can be activated by a clinician via a simple email to the care recipient, who can then access Connect RCM using their own web-connected device.1 Deploying the solution via the Internet eliminates the need for the shipment of hardware and can reduce deployment time from several days or weeks to just a few minutes. Best of all, it significantly simplifies the overall remote care plan program design and implementation process.
